Hello MPST Family:

All families are required to attend On-site registration.

Thursday, May 2^nd 7pm-8:30pm

or

Saturday, May 4^th 12noon-2pm

All New Swimmers must be prepared to tryout on either of the days above.

Come dressed in swim suit, cap, and goggles.

Items required for registration completion:

1. Online Registration Completed for All Swimmers.

2. Official photocopy of valid birth certificate; please bring the original and a copy for the team.

a.Not required if child was on any GCSL team 2018 Swim Season

3. Check payable to Mountain Park Swim Team (MPST) or online WePay payment for registration and merchandise fees.

4. 2019 MPA Center Concussion Statement of Agreement (one per swimmer)

5. $100.00 Volunteer Deposit (Separate Check)

6. Merchandise Orders Full Payment (On-line Orders are still available)

a. On website Click Swim Gear then 2019 Products

b. You may also add swim merchandise purchases at registration.

c. Note that merchandise orders will be submitted on May 10^th, so make sure that you have ordered all items desired.

7. Swim Suites can be ordered on Saturday, May 4^th, Swim and Tri will be onsite to do fittings. After Saturday, all swim suites will have to be ordered online.

Please take the time to download the required MPA Concussion Statement o f Agreement and have them pre-filled, for each swimmer, when you arrive at the pool for on-site registration.
