The Team Picture Day was scheduled on Monday, June 12 starting at 6:30 pm. Please see attached for the order information.   These need to be completed and brought to the vendor.  The vendor accepts cash, checks, and credit cards. Please fill out a form for each child and/or sibling photo that you would like to order.

Example: you want Package A for Jane Jones, Package B for John Jones, and an 8x10 of Jane and John Jones. you would need 3 forms. If you only want a sibling photo, it would be 1 form since you are not wanting individual of each child.
